Treating Stress Incontinence Without Surgery

Millions of women across the country are quietly suffering from stress incontinence. They wet themselves with the tiniest moves. Now a new procedure is giving them back the control they lost without invasive surgery. Coughing, laughing, sneezing. Three every day actions that trigger an unpleasant outcome for women with stress incontinence. "People drastically alter their daily life style. They stop going out shopping, they don't associate or socialize with their friends. It becomes a financial issue with purchasing absorption under garments, pads, liners," says Dr. Harold Tsai, a urologist on Lee Memorial Health System's medical staff. Loss of bladder control becomes a big source of embarrassment, one that leads many women to suffer in silence. For years the most advanced treatment was surgery; now there's a non-surgical way to curb the accidents. "You can monitor the temperature in each needle," says Dr.Tsai. Called the Renessa procedure, a catheter is placed at the base of the urethra. Using radiofrequency, heated needles are applied to the tissue in a series of minute long bursts. "They're curved little needles that we are able open and close. So when we push the instrument forward these needles come out and when we pull the button back the needles go back in," says Dr.Tsai. The procedure encourages the body to make scar tissue, which in turn toughens the area and provides better support and control of the bladder. "The procedure's 10 minutes, we do it under local anesthetic. Women leave the office usually within an hour and they're fully back to their daily activities the next day," says Dr.Tsai. The full affect takes three months. Studies show 60 percent of women find it fixed their problem, allowing them to regain control of their bladder and their life. Pregnancy, childbirth, hormonal changes during menopause, and anatomy of the urinary tract account for this difference. Urine is produced in the kidneys and stored in the urinary bladder. Urination is the process of emptying the bladder through the urethra that connects the urinary bladder to the external urethral orifice. There are two sphincters, or valves, that keep the urethra closed to prevent leak: internal urethral sphincter located at the neck of the bladder, and external urethral sphincter located right above the external urethral orifice and supported by the pelvic floor muscles. When the bladder is full, stretch receptors in the wall of the bladder send a signal to the spinal cord and the brain. At times when it's not appropriate to urinate, the brain sends back an inhibitory signal to keep the sphincters closed and prevent voiding. When you wish to urinate, this inhibition is removed; There are several types of urinary incontinence in women: -Stress incontinence: urine leakage while sneezing, coughing, laughing or any activity that creates abdominal pressure on the bladder. In women, this typically happens as a result of pregnancy and childbirth during which these muscles are overstretched. Stress incontinence symptoms usually worsen during certain times of the menstrual cycle when estrogen level is low. Incidents also increase following menopause. This is by far the most common type of incontinence in women. -Urge incontinence: need to void that cannot be deferred, inability to hold. This is commonly caused by an overactive bladder, a condition in which muscles in the wall of the bladder contract in an uncontrollable manner. The reason why this happens is unclear but it's likely to involve problems in the nervous system. -Overflow incontinence: constant dribbling of urine. Treatment depends on the type of incontinence and severity of symptoms. Lifestyle changes: - Limit fluid intake at certain times of the day - Cut down on caffeine, alcohol, keep a healthy weight. - Try pelvic floor muscle exercises such as Kegel exercises. - Timed voiding or bladder training therapy Medication: Depending on the cause of incontinence the following types of drugs maybe prescribed: - Anticholinergics: - For stress incontinence topical estrogen may be applied Non-surgical therapies for stress incontinence include: - pessary: - bulking agent injections Surgical procedures include: - Sacral nerve stimulation: A small pulse-generator device is implanted under the skin of the buttock; the device sends mild electrical impulses to the sacral nerve -the nerve that controls bladder activity- to moderate and control bladder spasms. - Bladder suspension or sling procedures may be performed to create an artificial support for the bladder neck or urethral sphincters. These procedures usually involve tightening of the bladder neck or the urethra to strong ligaments within the pelvis or to the pubic bones.

Urinary incontinence in women. Jeannine Miranne, MD, Division of Urogynecology at Brigham and Women’s Hospital, discusses urinary incontinence, which affects 17 percent of all American women. Risk factors for urinary incontinence include older age, childbirth, obesity, and menopausal status. Urinary incontinence is defined as involuntary loss of urine or a loss of urine beyond one's control. It can be divided into three main sub-types. Urgency incontinence is urine loss with a sudden urge or uncontrollable desire to void. Stress incontinence is urine loss with sneezing, coughing, laughing, and other physical activities that increase the pressure in the belly. Mixed incontinence occurs when a woman has symptoms of both urgency and stress incontinence. First line treatment for all types of urinary incontinence includes behavioral and dietary modifications: limiting fluid intake to 64 ounces daily and cutting down on foods and drinks that are irritating to the bladder, such as coffee, tea, soda, artificial sweeteners. In addition to behavioral and dietary modifications, there are other treatment options. Pelvic floor physical therapy can be recommended for women with urinary incontinence. This is particularly helpful in women who have difficulty identifying their pelvic floor muscles or in contracting them effectively. For stress incontinence, additional options for treatment include using a pessary, which is an inter-vaginal support device. Other treatment options for stress incontinence include peri-urethral bulking bulking agent injection, which involves injecting a filler in the urethra to bulk it up to help prevent urine loss, and surgery. Sling procedures can also be performed. The mainstay of treatment for urgency incontinence is medication to relax the bladder muscle so that it doesn't squeeze as often, preventing urine leakage. Additional options for the treatment of urgency incontinence include Botox injections in the bladder muscle and a procedure called InterStim, which involves placement of a device similar to a cardiac pacemaker. And the bladder can certainly drop," says Dr. Harold Tasi, urologist on Lee Memorial Health System's medical staff. Those pelvic floor muscles support the uterus, bladder, small intestine and rectum. Strengthening them can make the difference between getting to the bathroom or not. For years women have been told to use Kegel exercises. But it's easier said than done. "Everyone has a different idea how to do them, how many times to do them. It's very subjective. Now we're trying to get women in a very regimented way of doing pelvic floor stimulation," says Dr. Tsai. This device is helping Peterson. "Some women actually feel stimulation at a very low level," says Dr. Tsai. It's an insertable, electrical muscle stimulator that trains the body to tighten the pelvic floor. ETIOLOGY AND CAUSES Transient causes have a nice mnemonic - DIAPPERS Delirium - acute confusion Infections - UTI Atrophic vaginitis Psychological Excessive Urine - drinking too much fluids Restricted mobility Stool impaction Drugs that may cause temporary urinary incontinence Anti-cholinergics, anti-psychotics, anti-parkinsons, alpha agonist or alpha antagonist, Diuretics and ACE inhibitors. Calcium Channel Blockers, Sedative and Hypnotics. INVESTIGATIONS In the emergency room you must rule out cauda equina or cord compression. These will require MRI/CT and this is the only time you would want to do MRI/CT for urinary incontinence. Urinalysis and culture can rule out UTI. Urine cytology also helps rule out bladder cancer. Renal Function Tests are important because pathology may affect the renal function. Urodynamic studies such as uroflow studies (pressure in the bladder to the flow in urine). Postvoidal residual volume (amount of urine available after urinary and this may be done by ultrasound or catheter). Bladder complance showing how much bladder can hold. Cystourethroscopy is putting a camera into the bladder. STRESS INCONTINENCE Leakage with increased abdominal pressure such as cough or sneeze. Will have this classic presentation. In men the main cause is surgery (TURP) for prostate cancer. In women the causes are related to hypermobility of the sphincter since the external sphincter moves lower (usually because of vaginal delivery) and when the levator ani contracts with abdominal muscle and this can no longer occur since the levator ani is no longer surrounding the external sphincter. Sphincter deficiency is cuased by atrophy of vagina in postmenopausal women. Tend to improve with Estrogen. Inveatigations - Pad test is used to weight for 1hr or 24 hour to see how much is leaking. Cough Test - fill bladder and ask patient to cough and look for urine. Q Tip test consists of putting a Q-tip and ask patient to rest and strain. If Q Tip moves greater than 30 degrees then the patient has a hypermobile sphincter. Urodynamic testing may show low urethral pressure Treatment consist of weight loss, diet and Kiegle Exercises. Duloxetin (SNRI) can help symptoms. There are also surgical procedures. URGE INCONTINENCE Sudden feeling of urgency and before they make it to the bathroom they urinate on themselves.Due to detrosur overactivity and high sensitivity. Overactive bladder is they have urgency, frequency and may never have urge incontinence. Decrease bladder compliance. Causes consist of neurogenic causes, myogenic causes because of hypersensitive muscles. And idiopathic caueses. Treatment consists o bladder trianing to increase capacity. Hold urine to get bladder used to increase capacity. Public muscle training and relaxation of detrusor muscle. Fluid management to control how much water they drink. Dietary changes to decrease spicy food and citrus foods. Oxybutinin and botulinum to relax the sphincters. MIXED INCONTINENCE Patients have stress and urge incontinence. OVERFLOW INCONTINENCE Bladder distention due to detrusor or sphincter problem. Bladder outlet obstruction sch as BPH, neck contraction, urethral stricture, cystocele, pelvic organ prolapse. Decreased detrusor contraction by medications that decrease detrusor contraciton. Spinal lesions and long standing overdistention weakening muscles. Investigations to perform are postvoid residual volume measure amount of urine in bladder after voiding by ultrasound, catheter. Normal is less than 50mL and greater than 200mL is diagnostic. Anything in the middle is gray area. Treatment is to treat underlying cause if obstructive. Non-obstructive can be treated with Chronic Intermittent catheterization. NEUROPATHIC INCONTINENCE Due to lesions in brain, spine or peripheral nerve. Caused by strokes, trauma, aneurysms, hemmorrhage, MS. Spinal cord lesion and cauda syndrome. May cause decrease compliance by have high bladder pressure at low filling rates causing quick leaking. Common in meninomyelocele and Upper motor Neurons. Loss of sphincter tone by cauda equina, and open external sphincter. Investigations consists of neurologic, urodynamic, CT, MRI, and EMG. Treatment consists of increasing compliance with anti-cholinergics oxybutinin, imipramine, botulinum. Sphincter can be used with pessary to block the sphincter and when ready urinate the cork can be removed.
