Urination is the process of emptying the bladder through the urethra that connects the urinary bladder to the external urethral orifice. There are two sphincters, or valves, that keep the urethra closed to prevent leak: internal urethral sphincter located at the neck of the bladder, and external urethral sphincter located right above the external urethral orifice and is supported by the pelvic floor muscles. When the bladder is full, stretch receptors in the wall of the bladder send a signal to the spinal cord and the brain. At times when it's not appropriate to urinate, the brain sends back an inhibitory signal to keep the sphincters closed and prevent voiding. When you wish to urinate, this inhibition is removed, the spinal cord instructs the muscle of the bladder -- the detrusor muscle - to contract and the sphincters to open to let the urine out. In stress incontinence, small amount of urine leaks when the person is sneezing, coughing, laughing or having any activity that creates abdominal pressure on the bladder. This usually occurs because the pelvic floor muscles are weakened and can no longer support the sphincters. In women, this typically happens as a result of pregnancy, childbirth during which these muscles are overstretched. In most cases, this condition can be treated with exercises that strengthen pelvic floor muscles.

This is commonly caused by an overactive bladder, a condition in which muscles in the wall of the bladder contract in an uncontrollable manner. The reason why this happens is unclear but it's likely to involve problems in the nervous system. -Overflow incontinence: constant dribbling of urine. Treatment depends on the type of incontinence and severity of symptoms. Lifestyle changes: - Limit fluid intake at certain times of the day - Cut down on caffeine, alcohol, keep a healthy weight. - Try pelvic floor muscle exercises such as Kegel exercises. - Timed voiding or bladder training therapy Medication: Depending on the cause of incontinence the following types of drugs maybe prescribed: - Anticholinergics: - For stress incontinence topical estrogen may be applied Non-surgical therapies for stress incontinence include: - pessary: - bulking agent injections Surgical procedures include: - Sacral nerve stimulation: A small pulse-generator device is implanted under the skin of the buttock; the device sends mild electrical impulses to the sacral nerve -the nerve that controls bladder activity- to moderate and control bladder spasms. - Bladder suspension or sling procedures may be performed to create an artificial support for the bladder neck or urethral sphincters. These procedures usually involve tightening of the bladder neck or the urethra to strong ligaments within the pelvis or to the pubic bones.

Elidah, a small business funded by the National Science Foundation, created a medical device to help women with stress urinary incontinence - the inability to control the urge to urinate - a condition which impacts about one in three women. Urination is the process of emptying the bladder through the urethra that connects the urinary bladder to the external urethral o. There are two sphincters, or valves, that keep the urethra closed to prevent leak: internal urethral sphincter located at the neck of the bladder, and external sphincter located below the prostate gland and is supported by the pelvic floor muscles. When the bladder is full, stretch receptors in the wall of the bladder send a signal to the spinal cord and the brain. At times when it's not appropriate to urinate, the brain sends back an inhibitory signal to keep the sphincters closed and prevent voiding. When you wish to urinate, this inhibition is removed; the spinal cord instructs the muscle of the bladder to contract and the sphincters to open to let the urine out. Urinary incontinence is involuntary leakage of urine. There are several types of urinary incontinence in men: 1. Stress incontinence: urine leakage while sneezing, coughing, laughing or any activity that creates abdominal pressure on the bladder. This usually occurs because the muscles or tissues underneath the bladder are weakened and can no longer support it. In men, this typically happens after the prostate gland is removed for reasons such as prostate cancer. 2. Urge incontinence: need to void that cannot be deferred, inability to hold. This is commonly caused by an overactive bladder, a condition in which muscles in the wall of the bladder contract in an uncontrollable manner. The reason why this happens is unclear but it's likely to involve problems in the nervous system. 3. Overflow incontinence: constant dribbling of urine. This happens when the bladder does not empty properly while voiding making it almost always full and urine overflows. This may be due to weakened muscles in the bladder wall or a blocked or narrowed urethra. In men, the major cause for this type of incontinence is a condition called enlarged prostate or benign prostate hyperplasia, where the abnormal growth of prostate tissue compresses the urethra and blocks urine flow. Treatment depends on the type of incontinence and severity of symptoms. 1. Lifestyle changes - Limit fluid intake at certain times of the day - such as before going to bed or before a long trip. However, note should be taken to increase fiber content in your meals to prevent constipation. - Cut down on caffeine, alcohol, keep a healthy weight. - Try pelvic floor muscle exercises such as Kegel exercises. This is to strengthen the muscles that support your bladder. - Timed voiding or bladder training therapy: plan regular trips to the bathroom at set times of the day, gradually increase the interval between trips as you gain control. 2. Medication Depending on the cause of incontinence the following types of drugs maybe prescribed: - Alpha-blockers, 5-alpha reductase inhibitors: for treatment of enlarged prostate. - Imipramine, antispasmodics : these act on nerves to block bladder spasms in an overactive bladder. 3. Surgery Surgical procedures include: - Implantation of artificial sphincter: this is performed when weak sphincter is the source of the problem. - Male sling: implementation of an artificial support for the urethra. Urinary incontinence in women. Jeannine Miranne, MD, Division of Urogynecology at Brigham and Women’s Hospital, discusses urinary incontinence, which affects 17 percent of all American women. Risk factors for urinary incontinence include older age, childbirth, obesity, and menopausal status. Urinary incontinence is defined as involuntary loss of urine or a loss of urine beyond one's control. It can be divided into three main sub-types. Urgency incontinence is urine loss with a sudden urge or uncontrollable desire to void. Stress incontinence is urine loss with sneezing, coughing, laughing, and other physical activities that increase the pressure in the belly. Mixed incontinence occurs when a woman has symptoms of both urgency and stress incontinence. First line treatment for all types of urinary incontinence includes behavioral and dietary modifications: limiting fluid intake to 64 ounces daily and cutting down on foods and drinks that are irritating to the bladder, such as coffee, tea, soda, artificial sweeteners. In addition to behavioral and dietary modifications, there are other treatment options. Pelvic floor physical therapy can be recommended for women with urinary incontinence. This is particularly helpful in women who have difficulty identifying their pelvic floor muscles or in contracting them effectively. For stress incontinence, additional options for treatment include using a pessary, which is an inter-vaginal support device. Other treatment options for stress incontinence include peri-urethral bulking bulking agent injection, which involves injecting a filler in the urethra to bulk it up to help prevent urine loss, and surgery. Sling procedures can also be performed. The mainstay of treatment for urgency incontinence is medication to relax the bladder muscle so that it doesn't squeeze as often, preventing urine leakage. Additional options for the treatment of urgency incontinence include Botox injections in the bladder muscle and a procedure called InterStim, which involves placement of a device similar to a cardiac pacemaker.
